2023/2025 අධ්‍යයන වර්ෂ සදහා 12 ශ්‍රේණියට ශිෂ්‍යාවන් ඇතුළත් කිරීම සදහා අයදුම්පත්‍ර කැඳවීම

උපදෙස්

1. අයදුමපත් යොමු කළ හැක්කේ මාර්ගගත ක්‍රමය ඔස්සේ පමණි.

2. ඉහත සබැඳියට අදාළ මාර්ගගත ෆෝරමය නිවැරදිව පුරවා 2023 දෙසැම්බර් 09 වන දින 23:59 පැයට හෝ ඉන් පෙර යොමු කළ යුතුය.

3. එම ෆෝරමය සම්පුර්ණ කළ පසු එහි PDF පිටපත සුරුකිව තබා ගත යුතුය.

4. 2023 වර්ෂයේ පවත්වන ලද 2022 අ.පො.ස. (සා.පෙළ) ප්‍රතිඵල මත විශිෂ්ඨ සම්මාන නවයක් (9A’s) ලබා ගත් ශිෂ්‍යාවන් පමණක් අයදුම් කළ යුතුය. (මෙම නිර්ණායකය කලා අංශය යටතේ සෞන්දර්ය විෂයයන් සඳහා අයදුම් කිරීමේදී අදාළ නොවන අතර එම සෞන්දර්ය විෂය සඳහා විශිෂ්ඨ සාමාර්ථයක් සහිතව පළාත්, ජාතික හෝ ජාත්‍යන්තර මට්ටමේ ජයග්‍රහණ ලබා තිබීම විශේෂ සුදුසුකමක් ලෙස සලකනු ලැබේ.)

5. සම්මුඛ පරික්ෂණයට කැඳවිම සදහා සලකා බලනු ලබන්නේ ඉහත දිනයට හා වේලාවට පෙර අයදුම්පත් යොමු කළ හා මුලික සදුසුකම් සපුරාලන අයදුම්කරුවන් පමණි.

6. ඔබව සම්මුඛ පරික්ෂණය සදහා කැඳවනු ලැබුවහොත් පහත සදහන් සියලු ලිපි ලේඛන රැගෙන පැමිණිය යුතුය.

  • විදුහල්පති විසින් සහතික කරන ලද අයදුම්පතෙහි මුද්‍රිත පිටපතක් (PDF)
  • 2022 අ.පො.ස. (සා.පෙළ ) ප්‍රතිඵල ලේඛනයේ මුල් පිටපතක් සහ එහි සහතික කරන ලද ඡායා පියපතක්
  • උප්පැන්න සහතිකය, ග්‍රාම නිලධාරි සහතිකය, ජාතික හැදුනුම්පත සහ එම ලියවිලිවල සහතික කරන ලද ඡායා පිටපත්
  • 2020/2021/2022/2023 වර්ෂවලට අදාළව අයදුම්තෙහි සදහන් සුදුසුකම් තහවුරු කළ හැකි ලිපි ලේඛන වල මුල් පිටපත් සහ ඒවායෙහි සහතික කරන ලද ඡායා පිටපත්

7. කිසියම් හෝ ආකාරයකින් බලපැමක් සිදු කිරීම අයදුම්පත්‍රය ප්‍රතික්ෂේප වීමට හේතුවක් වනු ඇත.

 

Applications for Admission to grade 12 – Academic Years 2023/2025

1. Applications should be submitted only through online method.

2. Applications should be submitted using the link to online form  given above  on or before 23:59 hr, 9th December 2023.

3. Once the Google form is submitted, the PDF of the same should be saved and kept.

4. Only the applicants with nine distinction passes (9A’s) at the G.C.E. (O/L) Examination 2022, held in 2023 are eligible to apply. (This criterion is not applicable in the application of aesthetic subjects under the Arts Section, and having achieved provincial, national or international level achievements with a distinction pass in the said aesthetic subject is considered as a special qualification.)

5. Only the applicants who fulfill basic requirements and submitted the applications before the date and time mentioned above will be eligible to  called for an interview.

6. The following documents should be produced if you are called for an interview.

  • The printed copy of the application (PDF) certified by the Principal
  • Certified true copy of 2022 G.C.E. (O/L) certificate along with the original certificate
  • Copy of the Birth certificate, Grama Niladari certificate and NIC along with the originals
  • Certified photocopies and originals of all mentioned certificates of  achievements obtained during 2020/2021/2022/2023

7. Canvassing in any form will be a disqualification.
